I acknowledge that I am aware of the nature of the tryouts, practices and tournament participation and that playing lacrosse has certain risks of potential injury for those who participate.  I hereby release and discharge Hero’s Tournament Lacrosse Club, Inc., its agents, employees, officers, directors, coaches and other representatives from any claims, demands, responsibilities or liabilities, in law or in equity, for injuries or harm incurred as a result of my daughter’s participation in an activity as a member or potential member of a Hero’s team.  I authorize Hero’s Tournament Lacrosse Club, Inc., its agents, employees, officers, directors, coaches and other representatives to take whatever action they deem reasonably necessary, in their best judgment, in an emergency, and I hereby release and discharge Hero’s Tournament Lacrosse Club, Inc., its agents, employees, officers, directors, coaches and other representatives from any responsibility related thereto.
My daughter and I pledge to encourage good sportsmanship by demonstrating positive support for all players, coaches and officials.  We also pledge not to use profanity; criticize coaches, players or officials; touch an opposing player, coach or official in a threatening manner; or otherwise demonstrate poor sportsmanship at any event in which my daughter is participating as a member of Hero’s Tournament Lacrosse Club, Inc.  I acknowledge that my or my daughter’s failure to comply could exclude my daughter from being a participant in the Hero’s program or my daughter could be asked to leave prior to the conclusion of an activity.
I acknowledge that my daughter is a current member of U.S. Lacrosse and that I will make sure her membership remains current as long as she is a rostered player for Hero’s Tournament Lacrosse Club, Inc.
I am interested in volunteering to assist in the coaching, administrative or other activities of Hero’s Tournament Lacrosse Club, Inc.
